Job Description

*Qualifications:*

* Physical Therapy License or Physical Therapy Assistant License (Required)
* CPR Certification (Preferred)

*Pay:*

* Physical Therapist: $42.00 - $60.00 per hour DOE
* Physical Therapy Assistant: $30.00 - $35.00 per hour DOE

*Primary Responsibilities*:

* Provides physical therapy to home care patients and case management when indicated.
* Responsible for timely and accurate communication and documentation relative to physical therapy services.
* Promotes and develops physical therapy services in homecare*.*

*Specific Job Duties - includes, but not limited to:*

* Evaluate and treat patients suffering from physical disability due to injury, disease, or surgery in a home care environment.
* Performs initial start of care assessment and all related forms.
* Develop, revise and, execute an appropriate care plan for restorative and maintenance patients.
* Collaborate with physicians, case managers, nurses, occupational therapists, physical therapy assistants, home health aides, and office staff to ensure consistent levels of care.
* Monitor patient progress and document accordingly.
* Educate patients, families, and caregivers regarding the proper use of medical equipment or adaptive equipment.
