Job Description:
We are seeking an experienced LVN Wellness Director to join our team at our assisted living facility in Colma, California. This is an exciting opportunity to lead wellness programs, oversee patient care, and manage a dedicated team in a supportive, compassionate environment. The ideal candidate will have a passion for health promotion, patient education, and chronic disease management in an elderly care setting.

Responsibilities:

• Oversee wellness programs and services for residents
• Lead and mentor nursing staff in providing high-quality care
• Conduct patient assessments and develop individualized care plans
• Collaborate with physicians and multidisciplinary healthcare teams
• Educate residents on disease prevention, health promotion, and wellness practices
• Ensure compliance with healthcare regulations and facility protocols
• Be on call as necessary to provide support and address patient needs

Qualifications:

• Current LVN license (Must be an LVN)
• Experience in assisted living or long-term care preferred
• Knowledge of health promotion, disease prevention, and chronic disease management in elderly populations
• Excellent communication and team collaboration skills
• Ability to work independently and manage multiple tasks
