MTM Pharmacist - Help Patients Get the Most Out of Their Medications

Key Responsibilities

• Conduct comprehensive medication reviews (CMRs) with patients to identify medication-related problems.
• Provide targeted medication interventions (TMEs) and follow-up to support adherence and safety.
• Collaborate with prescribers, health plans, and care teams to resolve therapy issues.
• Document interventions clearly and maintain compliance with MTM program requirements.
• Educate patients on safe, effective use of their medications and address barriers to adherence.

What You'll Bring

• Education: Doctor of Pharmacy (PharmD) or Bachelor of Pharmacy degree.
• Licensure: Active and unrestricted pharmacist license in the U.S.
• Experience: Retail, hospital, ambulatory care, or prior MTM experience is a plus-but all licensed pharmacists are encouraged to apply, including new grads.
• Skills: Strong communication, patient counseling, and ability to work independently.
