Join Our Team as an RN Coordinator!

Position: RN Coordinator - Providence Home Services, Oregon

Schedule: Full Time / Days

Are you ready to make a difference in the lives of patients? As an RN Coordinator , you will play a vital role in planning and coordinating new referrals while providing essential phone triage for our existing patients. Most of your responsibilities will take place onsite, where you may also deliver direct nursing services and case management related to infusion therapy for patients in various settings, including their homes and clinics.

Your Responsibilities:

• Plan and coordinate new patient referrals.
• Provide phone triage for existing patients.
• Deliver direct nursing services and case management for infusion therapy.
• Collaborate with various providers, patients, caregivers, physicians, and payors.

What We’re Looking For:

Required Qualifications:

• Oregon Registered Nurse License (upon hire).
• Washington Registered Nurse License (upon hire).
• National Provider BLS certification from the American Heart Association (within 30 days of hire).
• 3 years of Home Infusion nursing experience.

Preferred Qualifications:

• Bachelor's Degree in Nursing.

Additional Information:

Pay Range: $46.54 - $72.26 (base pay; additional compensation may be available).

Work Location: Halsey Bldg - Home Service, East Portland, OR
