• Vascular experience with EVAR, TVAR, bypass, AAA, angiograms, carotids.
• Thoracic experience with robotics.
• Cardiovascular experience - 2 years CVS required.

Job Requirements

• FMOLHS MS Fingerprinting - State requirement for Mississippi.
• Proof of 2 immunizations, positive titer or declination accepted for MMR, Varicella.
• Proof of 3 immunizations, positive titer or declination accepted for Hepatitis B.
• Proof of vaccination within past 10 years or declination accepted for Tdap.
• Negative 10 Panel Drug Screen within 30 days prior to start.
• FMOLHS Skills Checklist - Unit specific to the contractor’s scope of practice within one year prior to start date; updated annually.
• FMOLHS Competency Exam - Copies of exams needed, within year of start with passing score of 80% or higher.
• FMOLHS Core Competencies - Must be current within a year prior to start with passing score of 80% or higher; updated annually.
• FMOLHS HRIS Form - See document library.
• FMOLHS Badge Photo - Forward facing, well lit, non-blurry photo with a neutral-colored background.
• FMOLHS Contract Annual Education Packet - See Document Library.

Additional Details

• Amsler Grid Vision Test required for onboarding.
• COVID-19 Vaccine proof or exemption accepted (optional).
• Fit Test must be one of the 3 mask fit types (N95- 3M 1860, 1860s or 1870+), completed within 1 year prior to start and updated annually.
• Influenza Vaccine required during flu season (10.1.2025 - 4.30.2026) with medical/religious exemptions accepted with proper documentation.
• TB Test proof of negative blood or skin test within required timeframes; if positive, MD note or proof of treatment required.
• Health Statement signed and dated within 1 year of start stating ability to work without restrictions.
• Annual TB Questionnaire required within 6 months of start with documentation if positive.
