Shifts:
Day Shift & On Call
Key Responsibilities:
Assess, develop, and implement individualized wound care treatment plans for residents.
Monitor and document progress of wounds, adjusting care plans as necessary.
Perform wound debridement, dressing changes, and infection control measures.
Collaborate with nursing staff, physicians, and interdisciplinary teams to ensure comprehensive patient care.
Educate staff, residents, and families on proper wound care, prevention techniques, and treatment procedures.
Ensure compliance with state, federal, and facility regulations regarding wound care.
Maintain accurate records and reports, documenting wound assessments and treatment outcomes.
Assist with pressure ulcer prevention programs and skin integrity assessments.
Participate in quality assurance initiatives to improve wound care outcomes.
Serve as a resource and mentor for nursing staff regarding best practices in wound management.
Qualifications:
Valid LPN or RN license in the state of employment.
Wound Care Certification (WCC, WOCN, or similar) preferred.
Prior experience in wound care, skin integrity, or related nursing field preferred.
Strong clinical assessment and critical-thinking skills.
Ability to work independently and collaborate with interdisciplinary teams.
Excellent documentation and communication skills.
