We're Hiring! We are hiring a full-time Health Coach to join us onsite in our newest clinic based in Los Altos, CA. This is an opportunity to join an interdisciplinary team committed to providing personalized, compassionate and high-quality SMI care. The Opportunity : Health Coaches at Amae are a critical part of our patient's care journey, supporting their physical health as well as their mental health. Reporting to the Clinic Director, the Health Coach is accountable for assessing clients' physical health, and developing comprehensive health plans that focus on lifestyle changes across the physical-mental health spectrum - inclusive of nutrition, exercise, sleep, financial health, relationship health, and treatment compliance. Via individual and group coaching sessions, our goal is to help patients maintain their treatment goals, and give them the tools to reduce stress and improve mindfulness, nutrition and medication adherence and increase physical exercise. You care deeply about coaching and teaching, and take great pride in seeing how health coaching can drastically improve the quality of life for complex patient populations. What You'll Work On : Patient-Centered Health Coaching Utilize evidence-based practices to provide health coaching to patients with severe mental illness in a clinical setting. Conduct behavioral assessments to understand patients' health and behavioral needs and develop tailored health coaching plans. Implement coaching plans using behavior change principles to identify and overcome patient barriers. Wellness & Physical Activity Facilitation Lead daily and weekly group activities that incorporate movement and physical wellness into care plans for our members. Organize and lead member outings, including hikes, tours, and sports activities. Coordinate with psychiatrists, therapists, and support teams to provide feedback on patient goal attainment and clinical issues. Support behavioral interventions related to dietitian / nutritionist-led dietary modification plans. Monitor and evaluate patient interventions and outcomes, making modifications as needed to support the patient's care journey. What You'll Have : 2+ years of experience in health education and / or health coaching, preferably in the mental health space BA / BS in appropriate field of specialization (examples such as Health Education, Exercise Physiology, Respiratory Therapy) Certification, advanced certification, and / or license appropriate to field of specialty as required Strong interpersonal skills and a demonstrated ability to work as part of a multidisciplinary team Solid understanding of basic workplace technology, and an interest in utilizing technology to improve outcomes for patients What We Provide : Employee Assistance Program 401(k) FSA & HSA savings programs Short & long-term disability Pre-tax commuter assistance program Paid time off Parental Leave - 12 weeks for birth and non birth parent 11 company holidays Amae Health is committed to fair and equitable compensation practices.
