A Shared Living Provider (SLP), is a community-based living option for individuals with an intellectual or developmental disability. A Shared Living provider supports a person who comes to live in their home with daily living skills, providing transportation to medical appointments, work, and getting out into the community. A Shared Living home becomes home for the person served.

The individual pays monthly room and board to the Shared Living provider. The subcontractor is paid through Goshen for support services covered by state funds allocated for residential services.

How can someone become a Shared Living provider?
Goshen looks for individuals that are caring, supportive and who have an extra room available in their home. It is preferred that the Shared Living provider have some experience with individuals with intellectual or developmental disabilities, but it is not a requirement. Applicants with backgrounds in human services, medical services, education, or counseling usually make great Shared Living providers.
Our goal is to create the best match possible between the subcontractor and the person desiring supports. Contractors must agree to a 12-month contract with the individual. However, it is hoped that the relationship will last much longer. Goshen ensures that the Shared Living provider meets all regulatory requirements, and provides training to provide quality services and supports.

What is Required to Become an SLP?

  • Must be 19 years of age or older
  • Complete an initial interview with Goshen staff
  • Valid drivers license and auto insurance with reliable transportation
  • Successful completion of background checks
  • Social Security Card
  • Proof of home owners insurance or renters insurance
  • Proof of current pet vaccinations
  • Certificates for any previously completed trainings
  • Physical exam and TB test
  • You have to be willing on taking behavior participants (Property damage, kicking, elopement , substance usage and more.. etc..)
  • Residence walk through to ensure a safe and comfortable environment
  • Complete necessary training
  • Strong desire to support another person to succeed and live a good life

What Training is Provided by Goshen?

  • CPR and First Aid
  • Medication Aide Training
  • MANDT Training
  • Orientation to supporting people with developmental disabilities
  • Documentation Training on the Therap database
  • Abuse, Neglect, and Exploitation training

Major Responsibilities of an Shared Living Provider

  • Provide a safe and supportive living environment
  • Complete daily documentation as required by Goshen and the State of NE
  • Administer medications and complete treatments as ordered by physician
  • Accompany the individual to doctors visits, providing support to make healthy lifestyle choices, and helping them to achieve a full and healthy life
  • Attend team meetings for the individual
  • Communicate with others involved in the individuals team regularly about their health and well being
  • Participate in a monthly check-in visit to your home by Goshen staff
  • Ensure the persons other needs are met, including hygiene, recreation, spiritual and social activities and involvement
  • Teach, support, protect and advocate for the person

Reimbursement to Shared Living Subcontractors
Goshen pays Shared Living subcontractors for providing habilitative services and supports. We offer a competitive rate. The contract amount is based on the persons individual authorized budget amount.
